What is dasatinib?

Dasatinib is a cancer medicine that slows the growth and spread of cancer cells in the body.

Dasatinib is used in adults and children to treat a type of blood cancer called Philadelphia chromosome-positive chronic myelogenous leukemia (CML).

Dasatinib is also used to treat Philadelphia chromosome-positive acute lymphoblastic leukemia (ALL) in adults and children.

Dasatinib can lower blood cells that help your body fight infections and help your blood to clot. You may get an infection or bleed more easily. Call your doctor if you have unusual bruising or bleeding, or signs of infection (fever, chills, body aches).

Some people using dasatinib have developed a rare but serious condition called pulmonary arterial hypertension (PAH). PAH may be irreversible if not promptly treated, and this condition can be fatal. Talk to your doctor about your specific risk of developing PAH.

Before taking this medicine

Some people using dasatinib have developed a rare but serious condition called pulmonary arterial hypertension (PAH). PAH occurs when blood pressure increases inside the arteries in your lungs. This makes it harder for your heart to pump blood through the lungs, which also weakens muscles in the heart. PAH may be irreversible if not promptly treated, and this condition can be fatal. Talk to your doctor about your specific risk of developing PAH.

To make sure dasatinib is safe for you, tell your doctor if you have ever had:

• a weak immune system (caused by disease or by using certain medicines);

• stomach or intestinal bleeding;

• liver disease;

• heart disease;

• low levels of potassium or magnesium in your blood;

• long QT syndrome (in you or a family member);

• lung disease; or

• lactose intolerance.

Dasatinib may harm an unborn baby. You may need a pregnancy test to make sure you are not pregnant. Use effective birth control while using dasatinib and for at least 30 days after your last dose. Tell your doctor if you become pregnant.

Pregnancy may be less likely to occur while the mother or the father is using dasatinib. Both men and women should still use birth control to prevent pregnancy because the medicine can harm an unborn baby.

CONTRAINDICATIONS

M  idodrine hydrochloride is contraindicated in patients with severe organic heart disease, acute renal disease, urinary retention, pheochromocytoma or thyrotoxicosis. M idodrine hydrochloride  should not be used in patients with persistent and excessive supine hypertension.

 

WARNINGS

Supine Hypertension: The most potentially serious adverse reaction associated with midodrine hydrochloride  therapy is marked elevation of supine arterial blood pressure (supine hypertension). Systolic pressure of about 200 mmHg were seen overall in about 13.4% of patients given 10 mg of midodrine hydrochloride. Systolic elevations of this degree were most likely to be observed in patients with relatively elevated pre-treatment systolic blood pressures (mean 170 mmHg). There is no experience in patients with initial supine systolic pressure above 180 mmHg, as those patients were excluded from the clinical trials. Use of midodrine hydrochloride in such patients is not recommended. Sitting blood pressures were also elevated by midodrine hydrochloride therapy. It is essential to monitor supine and sitting blood pressures in patients maintained on midodrine hydrochloride. Uncontrolled hypertension increases the risk of cardiovascular events, particularly stroke.

 

PRECAUTIONS

 

General: The potential for supine and sitting hypertension should be evaluated at the beginning of midodrine hydrochloride therapy. Supine hypertension can often be controlled by preventing the patient from becoming fully supine, i.e., sleeping with the head of the bed elevated. The patient should be cautioned to report symptoms of supine hypertension immediately. Symptoms may include cardiac awareness, pounding in the ears, headache, blurred vision, etc. The patient should be advised to discontinue the medication immediately if supine hypertension persists.

Blood pressure should be monitored carefully when midodrine hydrochloride is used concomitantly with other agents that cause vasoconstriction, such as phenylephrine, ephedrine, dihydroergotamine, phenylpropanolamine, or pseudoephedrine.
